Sneaking up on a Runner requires you to be behind them, however, as the game hints in loading screens, Runners will completely ignore your flashlight.

Bloaters can easily be killed with the Flamethrower too. Like Clickers, they will kill you once they get their hands on you. However, unlike Clickers, you can’t use a shiv to save yourself if you do get caught.
